In consideration of the First Baptist Church of Hamilton Park and/or the Annie Pearl Foster Center for Youth Enrichment agreeing to engage my child in learning activities, sponsored trips, and special projects, I/we hereby grant approval for his/her participation.

I/we understand that various forms of print and digital media will be a part of the learning activities, sponsored trips, and special projects of the First Baptist Church of Hamilton Park and/or the Annie Pearl Foster Center for Youth Enrichment. I/we grant permission for my child/myself and our likeness to be included or represented in print and digital media. I/we understand that these forms of media may be made available for public viewing and agree to participate without financial remuneration. I/we release the First Baptist Church of Hamilton Park and/or the Annie Pearl Foster Center for Youth Enrichment from any future claim or liability arising from the use of said print or digital media.

I/we understand that, I/we as the parent(s) or legal guardian(s) are responsible for the cost of medical treatment for my child if injured on the physical property of First Baptist Church of Hamilton Park or while participating in learning activities, sponsored trips, and special projects of the First Baptist Church of Hamilton Park and/or the Annie Pearl Foster Center for Youth Enrichment. Further, should the world situation or extenuating circumstances make it necessary for the administration of the First Baptist Church of Hamilton Park and/or the Annie Pearl Foster Center to cancel travel, or otherwise make alternative arrangements for learning activities and projects, parents and youth must understand that the no financial responsibility for any monies lost or encumbered due to these actions is assumed by the First Baptist Church of Hamilton Park and/or the Annie Pearl Foster Center for Youth Enrichment.

I/we consent to the release of health-related information to any personnel serving as sponsors/chaperones for the purpose of providing information necessary for the care and supervision of my child. In the event of an emergency while my child is engaged in learning activities, sponsored trips, and special projects, I/we hereby grant permission to employees and volunteers of the First Baptist Church of Hamilton Park and/or the Annie Pearl Foster Center to take whatever action is deemed necessary. In the event I cannot be reached, I authorize these individuals to give consent for my child to receive medical treatment.
